Direct Support Professional

As a Direct Support Professional at Sevita, you'll provide person-centered support to individuals with diverse abilities in home and community settings. Responsibilities

• Provide person-centered direct care and daily living support in home and community settings
• Support individuals with personal care, meals, and community outings
• Implement care plans, behavioral strategies, and skill-building activities
• Monitor, document, and report progress, concerns, and changes in condition
• Maintain a safe, respectful, and inclusive environment aligned with Sevita values
• Collaborate with families, clinicians, and teammates to coordinate services
• Participate in ongoing training and development to enhance quality of care
