As a Direct Support Program Coordinator at Sevita, you will lead and coordinate community-based services for individuals with disabilities and complex needs. Responsibilities

  • Coordinate daily operations of direct support programs serving individuals with disabilities and complex needs
  • Supervise and support direct care staff, ensuring person-centered, high-quality services
  • Develop, implement, and monitor individualized service plans in collaboration with interdisciplinary teams
  • Ensure compliance with agency policies, licensing, and regulatory standards
  • Collect and review documentation, incident reports, and progress notes for accuracy and timeliness
  • Communicate regularly with individuals served, families, and community partners
  • Support training, coaching, and performance feedback for team members
  • Assist with scheduling, staffing, and basic budgeting/resource allocation for programs
  • Promote a safe, inclusive, and respectful environment for individuals and staff
  • Participate in quality improvement initiatives and reporting
