Clinical Pharmacist

Intermountain Health seeks a Clinical Pharmacist to provide advanced, patient-centered medication management within our Healthcare & Medical Services department. Responsibilities

• Provide clinical pharmacy services including medication therapy management and pharmacokinetic dosing
• Review and verify medication orders for safety, accuracy, and evidence-based appropriateness
• Collaborate with physicians, nurses, and care teams on patient-centered treatment plans
• Educate patients and families on safe, effective medication use and adherence
• Participate in clinical rounding, care conferences, and transitions-of-care planning
• Monitor for and prevent adverse drug events, drug interactions, and duplications
• Support formulary management, antimicrobial stewardship, and medication-use policies
• Document clinical interventions and outcomes in the electronic medical record
• Precept or mentor pharmacy learners and contribute to staff education
• Engage in quality improvement, research, and initiatives that enhance patient outcomes
