Manager Social Services Case Management

Kaiser Permanente seeks a Manager Social Services Case Management to lead a team delivering high-quality, person-centered support to diverse members. In this role, you will oversee complex caseloads, design and refine case management workflows, and leverage digital tools to coordinate care across medical, behavioral health, and community resources. You will guide assessments, care planning, and referrals, ensure regulatory compliance and accurate documentation, analyze data to improve outcomes, and collaborate closely with clinical and IT partners to continuously enhance member experience and health results.

Responsibilities

• Lead and supervise a team of social services case managers supporting diverse member populations.
• Design and optimize case management workflows using Kaiser Permanente's digital and IT tools.
• Conduct complex psychosocial assessments and develop person-centered care plans.
• Coordinate services across medical, behavioral health, and community-based resources.
• Monitor caseload metrics, outcomes, and compliance with regulatory and accreditation standards.
• Use data and reporting tools to identify trends and drive continuous improvement.
• Provide case consultation, coaching, and training to case management staff.
• Collaborate with physicians, nurses, and IT partners to streamline referrals and care transitions.
• Ensure accurate, timely documentation in electronic case management systems and EHRs.
• Contribute to policy development, audits, and quality improvement initiatives.
