Manager Social Services

Kaiser Permanente seeks a Manager, Social Services to lead community and social services programs within our healthcare and medical services network. The Manager will oversee a multidisciplinary team delivering psychosocial assessments, care coordination, and discharge planning for diverse patient populations. Responsibilities include staff supervision, program development, quality and outcome monitoring, and collaboration with medical providers and community agencies. The ideal candidate blends clinical social work expertise with data-informed program management to improve access, equity, and health outcomes for vulnerable populations.

Responsibilities

• Lead and supervise social services staff across community and clinical settings
• Oversee psychosocial assessments, care coordination, and discharge planning
• Develop, implement, and refine social service programs aligned with healthcare goals
• Collaborate with physicians, nurses, and community partners to support patient care
• Monitor quality metrics, outcomes, and regulatory compliance
• Manage caseload priorities and resource allocation for high-risk patients
• Provide coaching, mentorship, and performance evaluations for team members
• Use data to identify service gaps and drive continuous improvement initiatives
• Ensure culturally responsive, trauma-informed, and equitable service delivery
• Participate in budgeting, reporting, and strategic planning for the department
