MDS Coordinator

Pine Point Center, a Genesis HealthCare facility in Dover, NH, seeks an MDS Coordinator, RN to lead resident assessment and care planning. You will manage the full MDS/RAI process, ensuring timely, accurate assessments, compliant documentation, and individualized, resident centered care plans. Collaborating closely with our interdisciplinary team, you'll analyze clinical data, support reimbursement accuracy, and drive quality outcomes. This on site role offers the opportunity to impact residents' comfort, safety, and independence while accessing education, resources, and growth across the Genesis network.
