LMSW Case Manager

Kaiser Permanente is seeking an LMSW Case Manager to join our Community & Social Services team within Healthcare & Medical Services. In this role, you will provide comprehensive psychosocial assessments, develop person-centered care plans, and coordinate medical, behavioral health, and community resources for diverse patient populations. You'll collaborate with interdisciplinary teams to reduce barriers to care, support chronic disease management, and promote patient self-advocacy. This position emphasizes data-informed practice, clear documentation in EMR systems, and continuous quality improvement in patient outcomes and service delivery.
