IRIS Consultant

Lutheran Social Services of WI & UP MI seeks an IRIS Consultant to support adults with disabilities and older adults in self-directed long-term care. In this role, you will help participants design person-centered service plans, manage IRIS budgets, and coordinate community-based supports that promote independence, safety, and quality of life. You'll conduct home visits, assess needs, document progress, and ensure program compliance. Working within a faith-inspired, trauma-informed, and inclusive culture, you will collaborate with families, providers, and internal teams to advocate for participants and connect them to vital resources.
