IRIS Program Consultant

Lutheran Social Services seeks an IRIS Program Consultant to support participants in self-directing long-term care services. In this role, you'll conduct person-centered assessments, develop individualized service and support plans, and educate participants and families on IRIS options and community resources. You'll coordinate providers, monitor budgets and outcomes, and ensure regulatory compliance while advocating for participant choice and dignity.
