Shared Living Provider

Mosaic Life Care seeks a compassionate Shared Living Provider to welcome an adult with support needs into a stable, caring home. You'll offer daily living assistance, medication support, and transportation while promoting independence, dignity, and community inclusion. In partnership with Mosaic's interdisciplinary team, you'll follow individualized care plans, document progress, and advocate for health and safety.
