Shared Living Provider

Mosaic Life Care is seeking a Shared Living Provider to offer compassionate, home-based supports to individuals with medical or developmental needs. In this role, you will welcome an individual into your home or provide consistent support in their home, assisting with daily living activities, promoting independence, and ensuring safety and dignity. You will collaborate closely with Mosaic Life Care's clinical team, follow individualized care plans, document progress, and support community integration.
