Position Overview

• Full-time outpatient NP role
• Medicare patient population
• Independent practice with physician collaboration
• Dedicated Medical Scribe support
• Reports to Center Medical Director

Responsibilities

• Provide routine and acute primary care visits
• Conduct annual wellness visits and HRAs
• Coordinate care with specialists and care teams
• Support population health and quality initiatives
• Patient education and care planning

Requirements

• MSN, DNP, or NP Certificate Program
• Board Certified NP
• Active NP license and DEA
• 2 years of primary care outpatient experience

Preferred

• HCC / ICD-10 / HRA familiarity
