Community Liaison

Addus Family Company seeks a Community Liaison to grow referrals for our home and community-based services. In this client-centered sales role, you will build and maintain relationships with hospitals, physicians, case managers, senior communities, and social service agencies. Responsibilities include promoting Addus services, conducting outreach and presentations, identifying referral opportunities, and coordinating smooth transitions to care. You'll collaborate with clinical and operations teams, track activities in a CRM, and represent Addus with professionalism and compassion while supporting our mission to help clients remain safely at home.
