A leading healthcare organization is seeking an experienced Medical Director Geriatric Medicine to oversee clinical operations across 11 PACE (Program of All-Inclusive Care for the Elderly) locations in the Philadelphia region. This leadership role combines administrative oversight with direct patient care, focusing on delivering comprehensive, community-based care to frail and elderly populations.

Position Details

Location: Philadelphia, PA
Position Type: Full-Time Permanent
Setting: PACE Program (Community-Based Care)
Coverage Area: Philadelphia, Delaware, Montgomery, and Chester Counties
Travel: Required between program sites, nursing homes, and participant centers
Call: Provider on-call rotation (typically two one-week assignments every 3 4 months)
Leadership: Supervision of PCPs and Nurse Case Managers

Key Responsibilities
  • Provide medical leadership and oversight for participant care

  • Lead and oversee the Quality Assurance Performance Improvement (QAPI) program

  • Develop and maintain medical policies, procedures, protocols, and standards

  • Supervise Primary Care Providers (PCPs) and Nurse Case Managers

  • Participate in interdisciplinary team meetings

  • Conduct participant intakes, assessments, and treatment planning

  • Lead advance care planning discussions with participants and caregivers

  • Coordinate care across multiple healthcare settings

  • Collaborate with consultants, pharmacists, and care teams

  • Ensure EMR optimization and documentation compliance

  • Participate in grievance resolution related to medical care

  • Respond to urgent and emergent participant needs

Compensation & Benefits

Compensation: $250,000 $290,000

Candidate Requirements
  • Active Pennsylvania Physician License

  • Board Certified in Internal Medicine or Family Medicine

  • Board Certification in Geriatrics Highly Preferred

  • CDS Certification and DEA Registration

  • Current BLS Certification

  • Previous supervisory experience required

  • Minimum one year of experience caring for frail or elderly patients

  • Strong chronic disease management and assessment skills

  • Comfortable working in an innovative, community-based care model

  • Ability to adapt to changing priorities

  • Proficiency with Microsoft Office

Highly Preferred
  • Additional geriatrics training or qualifications

  • Prior leadership experience in geriatric programs

  • PACE experience

Credentialing Requirements
  • Active Pennsylvania License

  • Board Certification details

  • DEA & CDS information

  • Documentation of geriatric experience

  • Supervisory experience details

  • NPDB Self-Query Report (within 30 days) Required at presentation
