Job Title: Primary Care Physician

Duration: 1 Years to long term
Shift: 8 hour/12 hour Shift
Job Responsibilities:.
• Direct Patient Care: The VA anticipates that the contracted provider(s) serving under this contract will spend an estimated 80% of their time involved in direct patient care.
• Scope of Care: The Contractor's Physician(s) (as appropriate and within scope of practice/privileging) shall be responsible for providing Primary Medicine Care promoted in a team-based, patient-centered care focusing on personalized, integrated, comprehensive, and coordinated approach to health care, including, but not limited to:
• Preventative Care: This includes annual physicals, screenings, and vaccinations to maintain good health and prevent disease.
• Acute Care: Treating common illnesses and minor injuries, such as colds, flu, rashes, and sprains.
• Chronic Disease Management: Helping patients manage long-term health conditions like hypertension, high cholesterol, and diabetes through regular check-ins, medication management and ordering tests.
• Health and Wellness: Providing counseling and patient education on healthy lifestyle choices, nutrition, and weight management.
• Diagnosis, Evaluation, Treatment, Care Management, and Consultation: Across all settings to include, but is not limited to care of: Chronic and acute conditions, Hypertension, Diabetes, Hyperlipidemia, Joint pain, Upper respiratory issues, Skin conditions, Mental health concerns, Other musculoskeletal issues, Neurology, Gynecology, Geriatrics, and Ambulatory conditions
• Clinic: The Contractor's provider(s) shall be present on time for any scheduled clinics as documented by physical presence in the clinic at the scheduled start time.Consultation and Referral: The Contractor's provider(s) shall provide consultation with and instruction to referring Physicians regarding appropriate indications for procedures so that the most expeditious and clinically appropriate work-up can be done. The Contractor's provider(s) shall determine the appropriate course of treatment and communicate in person or by phone with the referring clinicians. The Contractor's Physician(s) shall initiate appropriate social work referrals for all identified homeless veterans and for patients who do not have primary care providers.
• Medications: The Contractor's provider(s) shall follow all established medication policies and procedures. No sample medications shall be provided to patients.Discharge Education: The Contractor's provider(s) shall provide discharge education and follow up instructions that are coordinated with the next care setting for all patients.
Requirements:
• Active Physician License in any state of US
• Active BLS Certificate
• Active DEA
• Active Board certification
