• Modality: Primary Care
• Patient Population: Geriatric patients with multiple chronic conditions
• Patient Volume: patients per day
• Schedule: Monday Friday, 8:00am 5:00pm
• Hours: 40 hours per week (will consider PT)
• Contract Length: 6 months (open for extension)
• Start Date: ASAP
• Charting System: Athena
