Physician (MD/DO) Skilled Nursing Facility

Overview:
Our geriatric care group provides coordinated care across hospital and post-acute settings in the Fort Lauderdale area. Our physician will oversee skilled nursing facility patients, supervise clinical support staff, and maintain continuity with an established hospital-based patient panel. This role offers a flexible, part-time schedule focused on continuity of care.

Position Offers:

• Schedule: Flexible, 1 SNF day per week (provider-selected)
• Compensation: $5,000/month (DOE)
• Malpractice coverage provided
• Management of established private practice patients from HCA Woodmont
• Coordination of referrals to Tamarac SNF when appropriate
• Credentialing at HCA Woodmont required
• Privileges at Broward Health Coral Springs preferred

Key Responsibilities:

• Provide one onsite SNF day per week, including rounding and patient evaluations
• Supervise Nurse Practitioner in the SNF setting
• Manage patients from established hospital-based private practice panel at HCA Woodmont
• Coordinate appropriate referrals and transitions to post-acute facilities, including Tamarac SNF
• Provide ongoing evaluation and management of adult and geriatric patients
• Maintain continuity of care across hospital and SNF settings
• Complete accurate and timely clinical documentation
• Collaborate with interdisciplinary care teams as needed

Qualifications:

• MD or DO, board certified or board eligible in Internal Medicine or Family Medicine
• Active or eligible Florida medical license
• Must already be credentialed at HCA Woodmont; Broward Health Coral Springs privileges preferred
• Experience in SNF, post-acute, or inpatient geriatric care
• Strong communication and care coordination skills
