A nationally supported primary care organization is seeking a Board-Certified Internal Medicine Physician to join its outpatient practice in North Charleston, South Carolina.

Responsibilities

• Provide comprehensive outpatient primary care to an older-adult patient population
• Diagnose and manage acute and chronic medical conditions
• Develop individualized treatment and preventive-care plans
• Conduct routine examinations and health screenings
• Prescribe and manage medications
• Order and interpret appropriate laboratory and diagnostic testing
• Coordinate care with specialists, hospitals, and community resources
• Participate in interdisciplinary care-team meetings
• Maintain accurate and timely electronic medical records
• Support quality, population-health, and patient-outcome initiatives

Qualifications

• MD or DO from an accredited medical school
• Completion of an accredited Internal Medicine residency
• Board certification in Internal Medicine required
• Active or eligible South Carolina medical license
• Unrestricted DEA registration
• Outpatient primary care, geriatric, or value-based care experience preferred
• Strong communication and patient-engagement skills
• Commitment to collaborative, relationship-based medicine
