Key Requirements

• Licensure: MI license highly preferred
• Board Certified/Board Eligible: Required
• Certifications: BLS required
• Required Procedures: Radiologic Exam Interpretation, EMG, Botox Administration
• Vaccination: Required
• Credentialing Timeline: 120 days
