Job Summary
We are seeking a compassionate and experienced Hospice & Palliative Care Medical Director to provide clinical and administrative leadership for a growing hospice and palliative care program. This role focuses on delivering exceptional, patient-centered care, leading interdisciplinary teams, supporting quality initiatives, and partnering with providers to enhance hospice and palliative services.
Key Responsibilities- Provide medical leadership and oversight for hospice and palliative care services.
- Determine patient eligibility and certify/re-certify hospice and palliative care admissions.
- Collaborate with interdisciplinary teams to develop and oversee individualized care plans.
- Consult with attending physicians and specialists regarding patient care and symptom management.
- Conduct patient evaluations in home, outpatient, and inpatient settings as needed.
- Lead quality improvement initiatives and participate in clinical leadership committees.
- Educate clinical staff, physicians, and community partners on hospice and palliative care best practices.
- Support the development and growth of home-based palliative care services.
- Provide guidance on pain management, symptom control, and end-of-life care.
- Build relationships with referral sources and healthcare providers to expand program access.
- MD or DO with Board Certification/Board Eligibility in Hospice & Palliative Medicine .
- Board Certified in Internal Medicine or Family Medicine .
- Completion of a Palliative Medicine Fellowship preferred.
- Active, unrestricted New York State Medical License and DEA Registration .
- 1 2+ years of physician experience, including hospice, palliative care, pain management, or symptom management.
- Experience working within an interdisciplinary hospice or palliative care team.
- Excellent leadership, communication, and collaboration skills.
