Job Summary

We are seeking a compassionate and experienced Hospice & Palliative Care Medical Director to provide clinical and administrative leadership for a growing hospice and palliative care program. This role focuses on delivering exceptional, patient-centered care, leading interdisciplinary teams, supporting quality initiatives, and partnering with providers to enhance hospice and palliative services.

Key Responsibilities
  • Provide medical leadership and oversight for hospice and palliative care services.
  • Determine patient eligibility and certify/re-certify hospice and palliative care admissions.
  • Collaborate with interdisciplinary teams to develop and oversee individualized care plans.
  • Consult with attending physicians and specialists regarding patient care and symptom management.
  • Conduct patient evaluations in home, outpatient, and inpatient settings as needed.
  • Lead quality improvement initiatives and participate in clinical leadership committees.
  • Educate clinical staff, physicians, and community partners on hospice and palliative care best practices.
  • Support the development and growth of home-based palliative care services.
  • Provide guidance on pain management, symptom control, and end-of-life care.
  • Build relationships with referral sources and healthcare providers to expand program access.
Qualifications
  • MD or DO with Board Certification/Board Eligibility in Hospice & Palliative Medicine .
  • Board Certified in Internal Medicine or Family Medicine .
  • Completion of a Palliative Medicine Fellowship preferred.
  • Active, unrestricted New York State Medical License and DEA Registration .
  • 1 2+ years of physician experience, including hospice, palliative care, pain management, or symptom management.
  • Experience working within an interdisciplinary hospice or palliative care team.
  • Excellent leadership, communication, and collaboration skills.
