Responsibilities:

Provide direct patient care to residents in our skilled nursing facility, including administering medications, wound care, and vital signs monitoring

Develop and implement individualized care plans in collaboration with the interdisciplinary care team

Maintain accurate and comprehensive records of patient care and services

Assist with daily living activities, such as bathing, dressing, and feeding, as needed

Participate in quality improvement initiatives and contribute to a positive and supportive work environment
