Licensed Practical Nurse (LPN)

Hurst Plaza Nursing and Rehabilitation is looking for an experienced Licensed Practical Nurse (LPN) to join the long-term care unit at our beautiful skilled nursing facility. Duties and Responsibilities

Provide comprehensive Licensed Practical Nurse (LPN) care for residents

Administer medications and treatments as prescribed by the attending physician and in accordance with facility protocols

Monitor residents' health status and promptly report changes in condition to the supervising RN, physician, and appropriate family members or responsible parties

Provide skilled nursing services within scope of practice, including but not limited to tube feedings, suctioning, catheter care, wound care, dressing changes, and other ordered treatments.

Accurately complete all required documentation, charting, and reports in accordance with regulatory requirements and facility policies.

Support and implement individualized resident care plans while ensuring resident dignity, preferences, and rights are respected.

Observe residents closely and report significant changes in physical, mental, or emotional condition to the interdisciplinary care team

Collaborate with the interdisciplinary team to ensure coordinated, high-quality resident care

Skills and Qualifications

Active Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN) license issued by Texas required

BLS certification from the AHA required
