The Mission: Closing the GapYou will help build a clinical bridge for individuals transitioning from city and county jails back into the community.
You will provide:
- Medication management for high-acuity patients
- Forensic diversion support
- Integrated physical + psychiatric triage
- Continuity of care during critical transitions
This work directly supports some of the most vulnerable individuals in the system, ensuring stability where gaps often exist.

- No call, weekends, or after-hours responsibilities
- 90-minute new evaluations
- 30-minute follow-ups
- 12-15 patients/day
- Emphasis on clinical depth over volume

- Work in jail and community settings
- Build early rapport to improve outcomes
- Medical Director manages crisis calls
