Community Health Services Nurse Practitioner / Physician Assistant (NP/PA) Position Summary

The Community Health Services Nurse Practitioner or Physician Assistant provides comprehensive primary and preventive healthcare services to individuals and families within a community-based setting. This role focuses on improving access to care, managing acute and chronic conditions, promoting health equity, and delivering culturally competent, patient-centered care to diverse and underserved populations.

Essential Duties and Responsibilities
  • Provide comprehensive primary care services including health assessments, diagnosis, and treatment of acute and chronic conditions

  • Deliver preventive care services such as health screenings, immunizations, and wellness education

  • Manage chronic diseases including diabetes, hypertension, asthma, and cardiovascular conditions

  • Provide care coordination and referrals to specialty, behavioral health, and social support services

  • Address social determinants of health and connect patients with community resources

  • Promote health education, disease prevention, and lifestyle modification

  • Provide care across the lifespan as appropriate to scope of practice

  • Collaborate with physicians, nurses, care managers, social workers, and community partners

  • Maintain accurate, timely clinical documentation in the electronic health record

  • Participate in population health initiatives, outreach programs, and quality improvement activities

  • Support care transitions and continuity of care

  • Comply with all regulatory, accreditation, and professional standards

Qualifications
  • Graduate of an accredited Nurse Practitioner or Physician Assistant program

  • Current NP or PA certification and state licensure

  • Eligibility for credentialing and privileging

  • DEA registration or eligibility

  • Prior experience in community health, primary care, or public health preferred

Skills and Competencies
  • Strong primary care and clinical assessment skills

  • Knowledge of community-based healthcare delivery and population health

  • Ability to work with diverse and underserved populations

  • Excellent communication, cultural competency, and patient education skills

  • Ability to work collaboratively within interdisciplinary teams

  • Commitment to health equity, prevention, and patient-centered care

Work Environment
  • Community health centers, outpatient clinics, mobile health units, or public health facilities

  • May include extended hours, outreach activities, or limited call depending on program needs

Physical Requirements
  • Ability to sit, stand, and move throughout clinic and community settings

  • Manual dexterity for examinations and clinical procedures

  • Visual and auditory acuity sufficient for patient assessment and communication
