Nurse Practitioner - Geriatrics Fayetteville, North Carolina Key Candidate Requirements
- Licensure: Must be a NP licensed in North Carolina
- Experience: Minimum prior clinical experience required; fresh graduates not accepted
- Certifications: Current BLS/ACLS required
- Availability: Full-time, participation in on-call rotation
- Visa Status: PERM only; no locums or visa candidates
- Provide comprehensive, patient-centered primary care to geriatric and frail populations
- Collaborate with PACE Organization physicians to deliver primary care services
- Perform comprehensive participant assessments (initial, ongoing, as-needed)
- Develop, implement, and manage individualized care plans
- Evaluate and treat acute illnesses
- Participate in discharge planning for hospital or long-term care transitions
- Conduct home visits as needed for continuity of care
- Assist in developing and implementing clinical policies and procedures
- Actively participate in interdisciplinary team (IDT) meetings
- Ensure accurate, timely documentation of all clinical encounters
- Support a compassionate, mission-driven care environment
- Patient Population: Frail and elderly patients in a PACE (Program of All-Inclusive Care for the Elderly) setting
- Team: Collaborative interdisciplinary care team
