Internal Medicine Physician Assistant

Nurse Practitioner - Internal MedicineLocation: Peterborough, NH•Provide primary care services to patients across multiple clinical sites in a community-based setting •Collaborate closely with the Medical Director and Director of Medical Practice Operations to ensure high-quality, coordinated care •Serve as a key clinical provider within a network of primary care practices, supporting accessible and value-based healthcare delivery •Deliver patient-centered care through comprehensive assessments and development of individualized treatment plans •Diagnose and manage acute and chronic conditions, including prescribing medications and therapies as appropriate •Promote preventive healthcare through screenings, wellness visits, and patient education initiatives •Maintain accurate, timely, and compliant electronic health records (EHR documentation) •Work collaboratively with physicians, nurses, and interdisciplinary teams to optimize patient outcomes •Participate in quality improvement initiatives and clinical best practice development •Support a culture grounded in compassion, collaboration, honesty, and respect •Contribute to a team-oriented, community-focused healthcare environment •Perform additional duties as assigned Position Details:•Full-time role •Competitive salary based on experience •Supportive and collaborative work environment in a scenic New Hampshire community with access to outdoor activities, arts, and local amenities
