  • Joint Revision or Repair (Shoulder, Hip, Knee, Other)
  • Hand Surgery
  • Tendon Release/Repair
  • Peripheral Nerve Surgery
  • Spine Surgery with Instrumentation (Thoracic, Lumbar)
  • Spine Surgery without Instrumentation (Cervical, Thoracic, Lumbar)
  • Percutaneous Disc Surgery
• Credentialing: Estimated 120 days

Minimum Requirements

• Board Certified/Board Eligible: Board Certified required; Board Eligible minimum
• Certifications:
  • ACLS - Required
  • BLS - Required
• Licensure: Active Washington
