- Clinic (includes call): Monday - Tuesday, Wednesday, and Friday (8:00am-5:00pm)
- Surgical center (call only): Thursday (8:30am-4:00pm)
- Call - beeper, response time 15-30 minutes
- Weeknight call ( 5:00pm-8:00am)
- 24-hour weekend call (8:00am-8:00am)
- Also on call during clinic hours

- Licensed in Oregon
- BLS, DEA
