Hospice/Palliative Medicine Physician

Coverage Needed:

• Start Date: 10/12/2026
• Schedule:
  • Monday-Friday, 8:00am to 4:30pm
    • Night call, 4:30pm to 8:00am
  • Saturday- Sunday, 24 hr call
• Open Dates:
  • Monday Oct 12 (8am) - 19 (8am)
  • Monday Nov 9 (8am) - 16 (8am)
  • Potential week in December

Clinical Details:

• EMR: EPIC
• Trauma Level: III
• For hospice care only (no palliative)

Requirements:

• Able to cover most of the listed dates
• Case logs from the last 12 months (at time of submission)
• Active IN State License or IMLC
• Board Certified or Board Eligible (Certification must be obtained within 5 years of residency)
• Flu & Covid vaccinations: Required or Need Medical or religious exemption
• Certifications: BLS
