A well-established outpatient primary care practice in Columbia, South Carolina is seeking a Primary Care Physician to join its care team. This opportunity is ideal for a physician who enjoys building lasting patient relationships, working in a collaborative care model, and practicing medicine with a strong focus on quality, prevention, and continuity of care.

This is a full-time outpatient primary care position serving an adult patient population. The practice emphasizes chronic disease management, preventive care, medication management, wellness visits, and long-term patient engagement. Providers are supported by a team-based clinical structure designed to help deliver efficient, thoughtful, patient-centered care.

Job Responsibilities

Provide comprehensive outpatient primary care to adult patients
Manage chronic conditions such as diabetes, hypertension, COPD, CHF, and other common primary care needs
Conduct wellness visits, follow-ups, preventive screenings, and medication reviews
Collaborate with APPs, nurses, care managers, behavioral health, and other clinical support staff
Support a care model focused on quality outcomes, access, and continuity of care
Maintain accurate and timely documentation in the EMR

Qualifications

Board-certified or board-eligible Family Medicine or Internal Medicine Physician
Active South Carolina medical license, or ability to obtain
Primary care, internal medicine, family medicine, or geriatric care experience preferred
Comfortable managing adult and/or senior patient populations
Strong communication skills and a team-oriented approach to care
Commitment to preventive care, patient education, and long-term health outcomes

Full-time outpatient schedule
No nights
No weekends
No hospital rounding
