A well-established outpatient primary care practice in South Oklahoma City, Oklahoma is seeking a Primary Care Physician to join its care team. This opportunity is a strong fit for a physician who values relationship-based medicine, a consistent outpatient schedule, and a collaborative clinical environment focused on quality outcomes and long-term patient care.

This is a full-time outpatient primary care position serving an adult patient population. The practice emphasizes preventive care, chronic disease management, medication management, wellness visits, and continuity of care. Providers are supported by a team-based care model designed to help physicians deliver thoughtful, efficient, patient-centered care without the chaos of hospital rounding or after-hours call.

Job Responsibilities

Provide comprehensive outpatient primary care to adult patients
Manage chronic conditions including diabetes, hypertension, COPD, CHF, and other common primary care needs
Conduct wellness visits, follow-ups, preventive screenings, and medication reviews
Collaborate with APPs, nurses, care managers, behavioral health, and other clinical support staff
Support a care model focused on quality outcomes, prevention, access, and continuity of care
Maintain accurate and timely documentation in the EMR

Qualifications

Board-certified or board-eligible Family Medicine or Internal Medicine Physician
Active Oklahoma medical license, or ability to obtain
Primary care, internal medicine, family medicine, or geriatric care experience preferred
Comfortable caring for adult and/or senior patient populations
Strong communication skills and a collaborative approach to patient care
Commitment to preventive medicine, patient education, and long-term health outcomes
