A well-established outpatient primary care practice in Augusta, Georgia is seeking a Primary Care Physician to join its care team. This opportunity is ideal for a physician who wants a stable outpatient setting, a consistent schedule, and a collaborative care model focused on quality outcomes, prevention, and long-term patient relationships.

This is a full-time outpatient primary care position serving an adult patient population. The practice emphasizes chronic disease management, preventive care, wellness visits, medication management, and continuity of care. Providers are supported by a team-based clinical structure designed to help physicians practice efficiently while spending meaningful time with patients.

Job Responsibilities

Provide comprehensive outpatient primary care to adult patients
Manage chronic conditions including diabetes, hypertension, COPD, CHF, and other common primary care needs
Conduct wellness visits, follow-ups, preventive screenings, and medication reviews
Collaborate with APPs, nurses, care managers, behavioral health, and other clinical support staff
Support a care model focused on quality outcomes, access, prevention, and continuity of care
Maintain accurate and timely documentation in the EMR

Qualifications

Board-certified or board-eligible Family Medicine or Internal Medicine Physician
Active Georgia medical license, or ability to obtain
Primary care, internal medicine, family medicine, or geriatric care experience preferred
Comfortable caring for adult and/or senior patient populations
Strong communication skills and a team-oriented approach to care
Commitment to preventive medicine, patient education, and long-term health outcomes

Full-time outpatient schedule
