Practice Highlights / Qualifications

• Board Certified or Board Eligible in Internal Medicine or Family Medicine required
• Minimum three years of experience in senior care, primary care, or post-acute care preferred
• Outpatient-only practice focused on value-based, senior-centered care
• Work alongside APPs, RN care coordinators, social workers, and behavioral health professionals
• Opportunity to supervise advanced practice providers
• Experience managing complex patient populations and quality-driven metrics preferred
• Familiarity with value-based care, risk adjustment, and senior-heavy patient panels highly desired
• Clinical autonomy with strong support systems designed to improve patient outcomes
