Role Responsibilities

Provide thorough geriatric assessments and ongoing chronic care management
Diagnose and manage acute and chronic conditions common in older adults
Prescribe and manage medication therapies
Order and interpret labs and diagnostic studies
Coordinate care with physicians, specialists, and care teams
Maintain accurate and timely clinical documentation

Qualifications

️ Licensed Nurse Practitioner (FNP, AGNP, or GNP preferred)
️ Active or eligible Washington NP license
️ DEA certification (or eligibility)
️ Passion for delivering high-quality, evidence-based geriatric care
