- Effectively screen and evaluate patients with cognitive and ADL physical functioning disorders
- Develop and implement individualized treatment plans following all regulatory and clinical standards
- Utilize standardized assessments and evidence-based practices to support clinical interventions
- Provide rehabilitative, skilled, and medically necessary treatments including ADL retraining, therapeutic exercises, modalities, and assistive devices
- Monitor patient progress, adjust treatments, and evaluate outcomes to optimize recovery
- Collaborate with patients, families, caregivers, and healthcare team members regarding treatment plans and progress
- Document all services and outcomes accurately in electronic health records
- Participate in team meetings, trainings, and continuous education activities
- Supervise and support occupational therapy assistants, rehabilitation aides, and students as needed
- Valid Occupational Therapist license in Missouri
- NBCOT certification
- At least 1 year of experience in long-term care preferred
- Strong communication and team collaboration skills
- Ability to adapt treatment plans based on patient progress and referrals
