Shift: 2:00 PM - 10:00 PM
Schedule Hours: 36 hours/week
Rate: $1,422 / 36 hours

Requirements:

• Active LPN/LVN license in practicing state
• Experience in long-term care, skilled nursing, or assisted living preferred
• Strong clinical skills, accurate documentation, and effective communication
• Ability to follow care plans and collaborate with nursing staff

Responsibilities:

• Assist residents with activities of daily living and monitor health status
• Administer medications and treatments per RN/physician orders
• Accurately document care and report changes in condition to the healthcare team
• Maintain a safe, clean, and supportive environment
• Provide emotional support and companionship to residents
