Core Responsibilities and Essential Functions:
Patient Assessment:
- Collaborate with physicians to develop, evaluate, and update treatment programs, care plans, goals, and discharge plans based on the patient's physical therapy needs.
- Create and adjust care plans with input from the patient and/or family.
- Develop realistic discharge plans incorporating patient and family goals.
- Implement and adjust physical therapy treatment plans.
- Select appropriate activities to help patients achieve their goals.
- Revise goals and plans with input from patients and families.
- Provide ongoing education and training to patients and families.
- Give discharge instructions and referrals to community resources as needed.
- Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment.
- Document evaluations, treatment plans, and each session according to departmental guidelines.
- Complete and submit all required documentation, including family education and discharge information.
- Uphold ethical standards and demonstrate integrity and honesty.
- Embrace diversity and collaborate effectively.
- Maintain confidentiality of patient information.
- Adhere to attendance, punctuality, and dress code policies, including wearing an ID badge.
- Communicate clearly and effectively with all team members.
- Attend staff meetings, in-services, and other required activities.
- Organize workload to meet responsibilities timely and safely.
- Use materials and equipment safely and cost-effectively.
- Follow infection prevention protocols.
- Lead with vision, technology, innovation, and excellent customer service.

- Graduate from an accredited physical therapy program Required and
- Bachelor's Degree in Physical Therapy Required or
- Master's Degree in Physical Therapy Required or
- Doctorate in Physical Therapy Required

- Basic Life Support 2.00 Required
