Location:
Charlotte, NC (Anson & Mecklenburg Counties)
Travel between 4 skilled nursing facilities during scheduled shift
Schedule:
Full-Time 40 hours/week
Monday Friday 8:00 AM 5:00 PM
No nights. No weekends. No holidays. No on-call.

Overview:
Senior Community Care (SCC) program is a longitudinal, integrated care model designed to provide proactive, coordinated care to some of our highest-risk members residing in facility settings. As an Advanced Practice Clinician (APC), you ll work as part of an interdisciplinary care team delivering high-quality, patient-centered care while maintaining a true work-life balance.

Primary Responsibilities:
Conduct comprehensive medical assessments
Manage acute and chronic medical and behavioral health conditions
Ensure accurate, complete documentation including ICD-10 coding
Collaborate with interdisciplinary care team members
Lead advanced illness and advance care planning discussions
Provide patient and caregiver education and counseling
Required Qualifications:
Active, unrestricted NP or PA license in state of residence (or ability to obtain by start date)
Graduate of accredited NP or PA program

National board certification:
NP: AANP or ANCC (FNP, ANP, GNP, or AGACNP)
PA: NCCPA
Active DEA / prescriptive authority
Ability to meet physical requirements (lifting up to 30 lbs, navigating stairs, standing, kneeling)
Reliable transportation
Ability to obtain collaborative practice agreement if required by state
Full COVID-19 vaccination (per federal, state, and client requirements)
Ability to pass pre-employment drug screening
Preferred Qualifications:
1+ year of clinical experience (long-term care preferred)
Experience managing patients with complex medical, behavioral, social, or functional needs
Knowledge of geriatrics and chronic illness
Comfort with advanced illness and end-of-life conversations
Proficiency with EMR systems
