Mon 7am to 4pm

Tues 9am to 5pm

Wednes 7am to 2pm

Thurs 9am to 4pm

Fri 7am to 4pm

Details -Family/Employee Health Clinic

• Ages of children seen: 2+
• Outpatient NP/PA- Employee Health Clinic for employees and their families
• Ages 2 and up
• EMR- Athena

Requirements of the Family Medicine role:

• Active IN license
• 2 years of experience in a clinical setting
• Certifications BLS
• Active Board Certification
